Aims and background: Little is known about how participants perceive prevention trials, particularly trials designed to prevent mental illness. This study examined participants’ motives for participating in a trial and their views of randomisation and the ability to withdraw from a randomised controlled trial for prevention of depression. Methods: Participants were older adults reporting elevated depression symptoms living in urban and regional locations in Australia who had consented to participate in an RCT of interventions to prevent depression. Participants rated their (...) agreement with various statements describing motivations for enrolment in the trial and opinions regarding randomisation and withdrawal. Results: The majority of participants expressed a triad of altruistic motivation for participation, relative lack of concern about randomisation and commitment to the trial. Certain subgroups of participants, such as women and those with higher depression scores, reported higher levels of concern about specific issues. Conclusions: The findings suggest that participants enrolled in prevention trials for mental illness are likely to hold positive attitudes towards research trials. The identification of relationships between key person factors and trial-related attitudes enabled profiling of participant groups, which can inform recruitment strategies and interactions of participants and research projects in future prevention trials. Introduction: Although deep brain stimulation often improves levodopa-responsive gait symptoms, robust therapies for gait dysfunction from Parkinson's disease remain a major unmet need. Walking speed could represent a simple, integrated tool to assess DBS efficacy but is often not examined systematically or quantitatively during DBS programming. Here we investigate the reliability and functional significance of changes in gait by directional DBS in the subthalamic nucleus.Methods: Nineteen patients underwent unilateral subthalamic nucleus DBS surgery with an eight-contact directional lead in the most (...) severely affected hemisphere. They arrived off dopaminergic medications >12 h preoperatively and for device activation 1 month after surgery. We measured a comfortable walking speed using an instrumented walkway with DBS off and at each of 10 stimulation configurations at the midpoint of the therapeutic window. Repeated measures of ANOVA contrasted preoperative vs. maximum and minimum walking speeds across DBS configurations during device activation. Intraclass correlation coefficients examined walking speed reliability across the four trials within each DBS configuration. We also investigated whether changes in walking speed related to modification of step length vs. cadence with a one-sample t-test.Results: Mean comfortable walking speed improved significantly with DBS on vs. both DBS off and minimum speeds with DBS on. Pairwise comparisons showed no significant difference between DBS off and minimum comfortable walking speed with DBS on. Intraclass correlations were ≥0.949 within each condition. Changes in comfortable walk speed were conferred primarily by changes in step length.Conclusion: Acute assessment of walking speed is a reliable, clinically meaningful measure of gait function during DBS activation. Directional and circular unilateral subthalamic DBS in appropriate configurations elicit acute and clinically significant improvements in gait dysfunction related to PD. Next-generation directional DBS technologies have significant potential to enhance gait by individually tailoring stimulation parameters to optimize efficacy. (shrink)

This view accounts for some cases, But not for all. It is argued, In the note, That superficial, Factitious, 'got up' desires are judged insincere as such because not 'natural' to a man. This is implied by the claim of andre gide's which walker misinterprets. In order to understand the various strands of general equilibrium theory, why it has taken the forms that it has since the time of Léon Walras, and to appreciate fully a view of the state of general equilibrium theorising, it is essential to understand Walras's work and examine its influence. The first section of this book accordingly examines the foundations of Walras's work. These include his philosophical and methodological approach to economic modelling, his views on human nature, and the basic (...) components of his general equilibrium models. The second section examines how the influence of his ideas has been manifested in the theorising of his successors, surveying the models of theorists such as H. L. Moore, Vilfredo Pareto, Knut Wicksell, Gustav Cassel, Abraham Wald, John von Neumann, J. R. Hicks, Kenneth Arrow, and Gerard Debreu. The Aquinas Lecture is given annually at Marquette University under the auspices of the Philosophy Department. It was initiated in 1933 by the Rev. George H. The long-term persistence of neurotic symptoms, such as anxiety, poses difficult problems for any psychological theory. An attempt is made to revive the Watson-Mowrer conditioning theory and to avoid the many criticisms directed against it in the past. It is suggested that recent research has produced changes in learning theory that can be used to render this possible. In the first place, the doctrine of equipotentiality has been shown to be wrong, and some such concept as Seligman's “preparedness” is required, (...) that is the notion that certain CS are biologically prepared to be more readily connected with anxiety responses than others. In the second place, the law of extinction has to be amended, and the law of incubation or enhancement added, according to which the exposure of the CS-only may, under certain specified conditions, have the effect of increasing the strength of the CR, rather than reducing it. The major conditions favouring incubation are Pavlovian B conditioning, that is a type of conditioning in which the CR is a drive; a strong UCS, and short exposure of the CS-only. (shrink)

An increasingly number of patients in Western civilizations suffer from weak authenticity structure which is characterized by a lack of self-realization, autonomy, character strength, stereotype behavior, inability to use (internal) dialogue in order to learn about oneself and defining oneself as a individual, and so on. In this paper a theoretical model of fragile authenticity structure and some suggestions to regain a more authentic attitude are presented.

Sct. 1 accepts Rundle's point that ‘meaning’ and ‘use’ are used differently, but insists that this is compatible with holding that use determines meaning, an therefore holds the key to conceptual analysis. Scts. 2–4 rebut three lines of argument which claim that linguistic philosophy goes astray by reading into the meaning of words non‐semantic features of its use: Searle's general speech act fallacy charge, Hacker's (...) use of the Frege‐point against Wittgenstein's account of avowals, and Grice's attack on Wittgenstein's discussion of ‘trying’. Sct. 5 argues that Grice's doctrine of conversational implicature fails to show that the features he disregards are pragmatic rather than semantic. Abstract.Growing interest in the origin of life, the physical foundations of biological theory, and the evolution of animal social systems has led to increasing efforts to understand the processes by which elements or living systems at one level of organizational complexity combine to form stable systems of higher order. J. Bronowski saw the need to extend or reformulate evolutionary theory to deal with the hierarchy problem and to account for the evolution of systems of “stratified stability.” The hierarchy problem has (...) become a matter of great interest also in nonequilibrium thermodynamic theory.An effort is made here to develop an abstract, phenomenological model, based on the laws of thermodynamics, to account for the origin and hierarchical evolution of living systems. It is argued that the principle of minimum entropy production, developed by I. Prigogine, applies generally to all thermodynamic systems and processes and is implicit in an extended and more complete formulation of the second law of thermodynamics. From this are derived a thermodynamic criterion and a principle of thermodynamic selection governing the formation of stable systems of “elements” of various levels of organization. Thermodynamic selection gives rise to the creation of “elements” having increasingly “open” characteristic structures which may combine spontaneously to form “social” or crystalline systems capable of growing and reproducing themselves through processes of fissioning or budding. Such simple, self‐reproducing systems are capable of evolving by natural selection, which is seen to be a special case of the more general process of thermodynamic selection. The principle of natural selection, thus formulated, has the character of a fundamental physical law. Self‐reproducing systems with suitably open hereditary programs may combine to form stable social systems, which may grow and reproduce as a unit. In this way self‐reproducing systems of increasing hierarchical order, size, and organizational complexity may evolve through processes of thermodynamic (natural) selection. Some implications of this open‐ended model and opportunities for testing its empirical and theoretical utility are explored. (shrink)
